Eligibility Criteria
To ensure we are serving those who need it most, patients must meet all three of the following criteria. Eligibility is verified at each visit.
- No Health Insurance
- Individuals must currently be uninsured. Those with Medicaid, Medicare, or any private insurance do not qualify. If your insurance is pending or lapsed, we can offer bridge care until a new healthcare provider is established.
- Age Eligibility
- Medical services: ages 19–64
- Dental services: ages 16 and older
- Household Income at or Below 200% of the Federal Poverty Level
- If you are unsure whether you qualify, please contact us. Proof of income may be requested at any time. Please see the chart below for current guidelines.
The Rural Parish Clinic reserves the right to verify eligibility at any time and to decline services to individuals who do not meet all criteria. Eligibility does not guarantee the availability of a specific appointment, service, or provider. Patients who do not qualify may be referred to other community health resources.
